About the role
Visit eight to ten homebound patients per week across Passaic and Clifton, managing care plans and family education.
Responsibilities
- Complete in-home assessments and update care plans
- Coordinate with physicians and family caregivers
- Document visits in the agency EMR the same day
Requirements
- Active NJ RN license
- 2+ years clinical experience
- Reliable vehicle
